Objective: To investigate final histological outcomes of patients with cervical intraepithelial neoplasia (CIN) grades 2 and 3 and positive surgical margins following loop electrosurgical excision procedures.
Material and Methods: This study included patients with CIN grades 2 and 3 and positive surgical margins following loop electrosurgical excision procedures (LEEP), who had undergone additional surgery between 2007 and 2014 at Baskent University Hospital in Ankara. Clinical features and the results of cytology, colposcopy and pathology were collected from hospital records.
Results: 104; 75 with CIN 2 and 29 with CIN 3, underwent repeat surgical procedures and were included in this study. Median patient age was 37 years (25th-75th percentiles: 33-42 years), 70.2% had at least one child and 86.5% were premenopausal. Of the 104 patients, 78 underwent repeat LEEP and 26 underwent simple hysterectomy. Subsequent surgical pathology showed that 37 (35.6%) patients had normal results, six (5.8%) patients had CIN 1, 44 (42.3%) patients had CIN 2, 13 (12.5%) patients had CIN 3 and four (3.8%) patients had microinvasive squamous cervical cancer.
Conclusion: In patients with CIN grades 2 and 3 and positive surgical margins the absence of any CIN lesion has been frequently observed however there is a risk of undiagnosed cervical cancer in following subsequent operations.
